The easy way!
Jim has printed out a blank set-up sheet, hand-written it, then scanned it and uploaded it to the set-up forum. If you have access to a scanner, I'd do that, or hand it to me and I'll scan it for you!
The hard way
I would use a graphic program, such as Corel Draw, to add the details to the set-up sheet then save the file as a jpeg. I would then just add the set-up sheet image to a post.
To open the blank pdf set-up sheet in your graphic program, first open the set-up sheet using Acrobat.
Then use the snapshot tool (looks like a little camera) and cut out the whole set-up sheet, you will get the message 'The selected area has been copied to the clipboard'
Now open your graphic program, then select paste. The set-up sheet will now be pasted and will be on your screen ready to edit. Then simply place text & colour in the relevent boxes. When you have filled it in, simply save the file as a jpg then add to a new post on this web site. The file needs to be under 2mb in size.
If you are having problems, you can always just send me your set-up info and I will fill in the sheet and email to you so you can post it.
